Report of Trouble 

A report of trouble is a document that informs about a trouble or a hindrance in the operation of a machine or in completion of a work, routine job or a project or the like. It explains the nature of the problem or hindrance and possible solution also. It may be presented in memo or letter or manuscript format depending upon the nature of the problem. 

Nature of the problem may vary depending upon the context. It may be some functional issue of mechanical, electrical, electronic type or a bug. It may be an issue raised by a customer in service industry. It may be an issue resulting from the outside environment. 
Formats to Present a Trouble Report
1. Interoffice Memo. 
When the trouble report is presented to an office holder within the organization a memo is the best choice. Remember that you must follow the practice prevalent in your organization. 
2. Templets. 
Some organizations use readymade templets for such reports. Follow the practice of your organization.
3. Letter. 
A trouble report is sent via letter when the recipient of the report lies outstation. 
4. Manuscript. 
A manuscript format is used when the issue is highly persistent and requires inspection of several dimensions involving great expertise from outstation experts and the issue requires detailed analysis and inspection.

Elements of a Report of Trouble
1. Description of the Problem. 
2. Severity of the Problem
3. Impact of the Problem
4 Solution of the problem
If the solution is available, solve it and keep the record.
5. If you don't have the solution, tell the solution and request the concerned authority to get the solution carried out. Keep the record.

Subject: Report of trouble by ongoing strike

As per our telephonic conversation yesterday, I tried to establish communication with Mr. Banwari Lal, the leader of the Labour Union, expressed willingness to talk, supported by our willingness to listen, I invited him to come on the table and have conversation with me, regarding the problem of labourers but I regret to inform you that it did not work. Then I proposed him that we are ready to pay the double rate of overtime wages but it also did not work. According to my trustworthy sources, the matter has been hijacked by Samajwadi leader Mr. Vidya Charan Dubey. He is the person who is fueling the fire of strike to prove himself the Messiah of labourers. He has misguided them by offering his support for higher wages. I tried to contact him but he did not pick up my phone. Thus hereby, I present here the report of trouble that I faced today.

I think I need your support in this regard.
